In 2023, 23% of individuals experiencing homelessness in the state had a severe mental illness,* and 21% had a chronic substance use disorder.* With a poverty rate of 15.6%,* the eighth highest in the nation, and over 20% of children living in poverty,* many families struggle to access essential services. Additionally, Oklahoma has one of the highest incarceration rates in the U.S.,* disproportionately affecting low-income and minority communities. Community-based case management is vital in bridging these gaps, providing personalized support to help individuals navigate complex systems, access resources, and achieve stability.
